INTRODUCTION

A problem is defined as a gap between the standard and the reality. Fundamentally, when there is such a gap, we search for the root cause and implement countermeasures to prevent the recurrence of the problem.

Fundamental to any continuous improvement program is the ability to determine the root cause of an effect. 

Root cause analysis involves - deploying methodologies to understand the root cause(s) of a problem and verify the same.

Problem-solving tools and techniques are universally deployed approaches, that are structured to identify and verify the root cause in a scientific manner, followed by implementing countermeasures.
